ACS provides patient education on medication, complex health conditions, ongoing support and calls to doctors, specialist, physical therapy, speech, respiratory. We work with the patient and/or the family depending on the ability of the patient to advocate for him/herself. Our mission is to produces verifiable health improvements in our patients. We provide quarterly reports with the measurable differences our services make in the lives of their loved one. Services can start/stop upon request.
