TCM Coordinates medical, mental, or financial care/resources

Targeted Case Management (TCM) provides intensive, person-centered care coordination for individuals with serious mental illness (SMI), substance use disorders (SUD), and complex physical health conditions. We coordinate medical, behavioral health, housing, and social services to ensure continuity of care and prevent gaps that lead to crisis. We go beyond referrals—offering hands-on advocacy, transportation coordination, benefit support, and real-time problem solving to remove barriers and stabilize lives. Outcomes include improved appointment adherence, increased treatment engagement, and housing stability (90+ days), while reducing avoidable emergency room visits, hospitalizations, and overall Medicaid costs.
