**What is Coach Vera’s Slow & Steady Health Program?**

Coach Vera’s Slow & Steady Health Program is a CDC NDPP certified diabetes prevention program serving adults in NYC. Our mission is to prevent type 2 Diabetes by educating adults at risk and supporting sustainable lifestyle changes in nutrition and fitness. We offer a yearlong group program, monthly in-person meetups, and real-life lectures, group discussions, and peer support.

**Who is the program intended for?**

The program is intended for adults (18+) in NYC who are prediabetic or at risk. If you are pregnant or already diagnosed with diabetes, this program might not be suitable for you. We encourage all adults at risk to join.

**What makes Coach Vera’s Slow & Steady Health Program different?**

Our program is CDC NDPP certified and focuses on real-life scenarios and peer discussion. We have five trained Lifestyle Coaches who provide a mix of virtual and in-person support. We aim to provide clarity, empathy, and outcome-oriented advice without pressuring our participants.

**How does the program work?**

The program runs for a year and includes weekly then biweekly Zoom meetings for group sessions. We also offer optional monthly in-person meetups. Our sessions involve real-life lectures, group discussions, and peer support. Participants will also have goals like 5% weight loss target and 30 minutes per day activity goal for 5 days a week.

You can join our program by leaving your contact information and pre-diabetes survey results at coachvera55@gmail.com. Our team will then reach out to you for a supportive onboarding follow-up. We do not require a copay, but we do require medical insurance info. We respect and enforce HIPAA privacy rules.
