What you'll need to get your Wisconsin license:
- Your Social security number (SSN) - if you have one.
- Proof of U.S. citizenship, legal permanent resident status, conditional resident status or temporary visitor status, proof of name and date of birth, proof of identity, proof of Wisconsin residency if over 18.
- If you are under age 18, complete the section of the application for applicants under age 18 (applicant, school and sponsorship certifications).
- If you are applying for a commercial driver license, complete the section of the application for commercial driver applicants
- $35 Instruction permit (Class D); $35 Instruction permit renewal (12 month); $28 Original probationary (Class D) (expires 2 years from next birthday); $34 Original regular (Class D)(8 year); $15 Skills exam (Class D) – charged for every appointment not cancelled 24 hours in advance
Wisconsin DMV Driver License: What You’ll Do
- Comply with the licensing requirements and complete the Wisconsin Driver License application MV3001.
- Apply for your Learner’s Permit: submit the required documents and ID at the DMV, pass the written and vision test, receive your Learner’s Permit.; consider making a DMV appointment or getting a line waiting concierge.
- Practice driving to prepare for your road test by enrolling in a driver’s education course or, completing 30 hours of supervised driving, including 10 hours at night (must be accompanied by a front-seat passenger who is at least 21 years old). If you're over 18 you won't need the required hours.

- Visit the DOT to take the road test. Once you pass, you will receive a temporary driver’s license then you'll receive your new Class D driver’s license via U.S. mail
Here's a Wisconsin driver's license exceptions to note:
- If you have been convicted of an alcohol/drug related offense in another state, you are required to submit proof of compliance of an assessment or program plan when applying for a WI Driver License. Proof of compliance may be required for all offenses that have occurred within the last 3 ½ years. Failure to submit proof of compliance will result in a loss of your WI driving privilege whether or not you have a valid license from another state. You will not be able to reinstate your WI driving privilege until you provide us with proof of your compliance.
- For some medical conditions, you will be asked to have your doctor provide medical information.
