What You’ll Need to Renew your Wisconsin Driver License by Mail
You can only mail in your renewal application under the following conditions:
- You are not currently in the state of Wisconsin.
- You must be a U.S. citizen.
- The Wisconsin DOT must already have your Social Security Number in their system.
- Your drivers license cannot have been expired over eight (8) years ago.
- Make sure that you meet the When to Renew requirements.
- You may only use this by mail feature if you last renewed in person.

- Visit your nearest DMV office in person. Consider making a DMV appointment or getting a line waiting concierge.
- Complete and download the following MV3001 Wisconsin Driver License form. You will need to complete this form in order to renew.
- Submit the required documents and surrender your license.
- A vision test will be required.
- Bring either Cash or a Check to pay the Fee of $34.00. Add $5.00 if you are renewing late.

- Complete and download the following MV3001 Wisconsin Driver License form. (Make sure to complete every section of the application. This includes your name, date of birth, social security number, Wisconsin address, out of state address, gender, race, eye color, hair color, weight and height).
- You must have a vision test. An eye care professional must complete the Vision section of the application or have him/her complete the following MV3030V Vision Examination form. Note: The exam cannot be older than ninety (90) days.
- The contents of your mail packet must include photocopies of the following items: Proof of Residency document. It must show your name and Wisconsin address. Please use any of the following documents (your current expiring drivers license cannot be used for this): Utility bill, mobile phone bill, bank statement, mortgage or rental agreements, homeowner's insurance, hunting or fishing license; Proof of identity document. Please use any of the following documents: your current or expired drivers license, ID card, marriage certificate, social security card, school ID, etc.
- Make payment for the Fee of $34.00. Add $5.00 if you are renewing late. The only forms of payment are Checks or Money Orders.
